I somewhat expected this

based on the builtin\builtin. The NSL service seems to be getting no user name and password for the user.

I am sorry I didn't ask you to also do this.
    Goto Windows Control Panel/System/Advanced/Environment Variables/System Variables

    add DEBUG_NPNOTES with a value of 1.

    Reboot and have the user that fails log in. Once logged in look for:

    npnotes.log - this file is placed in the temp directory as set in the system environment (defined in the following order %TMP%, %TEMP% or %WINDIR%)

    and provide that file. It is important to note that this is a system variable not a user variable (and not Notes.ini variable).

    Don't forget to turn of the DEBUG_NPNOTES.
